Creating Online Forms with Formassembly
Simple Forms
When creating simple online forms for the university, FormAssembly is used. FormAssembly is a web-based form generator where the submitted information can be gathered via email and/or exported to spreadsheet format. For more information on using FormAssembly, please refer to the university the CMS Webguide on topics related to FormAssembly and other web development-related tutorials.
To get to the CMS Webguide, searchWebguide on the University homepage, not the Helpdesk homepage, and click CMS Webguide (University of Michigan – Flint). There will be a blue column on the left hand side with a list of CMS Webguide topics, including FormAssembly.

Forms Requiring a Signature
When your form requires a physical signature, PDF is the best format. Please contact ITS Training at ITStraining@umflint.eduor 810-762-3123 for assistance to create a PDF which allows the information to be typed in, and then the form can be printed and signed.
Advanced Forms
If your form requires more advanced capabilities, the ITS Webmaster will be able to guide you with creating a web-based information-gathering system. You can contact the Webmaster at webmaster@umflint.edu or by calling ITS at 810-762-3123.
