In order to be considered as part of the applicant pool, you will need to fully complete and forward each document listed below to the address on the form.  This is the second part of a two-step application process to our DPT program.  If you are just beginning the application process, please visit the Graduate Admissions page for complete instructions.   

For your convenience, you may choose the Microsoft Word format or PDF file.  An Adobe Acrobat Reader is necessary to view the PDF file option.  If you don't have a reader, you can download one for free by clicking the icon below.

CPR and First Aid certifications are also part of the application process,
however, those are not required by the deadline stated below.

All completed application documents listed below must be received 
ON OR BEFORE the deadline to be considered for the applicant pool. 

DEADLINES:
Early Admissions:  NOVEMBER 15TH
Standard Admissions:  JANUARY 15TH
