Application Forms
In order to be considered as part of the applicant pool, you will need to fully complete and forward a Clinical Observation Form to the address on the form. This form is available on the PTCAS website. This is the second part of a two-step application process to our DPT program. For your convenience, if you have already utilitzed the form from our website you may do so. However, please be advised that you must include your PTCAS ID number on the form prior to submission.
