Practice Services, Fees. and Hours at the UHWC

UM-Flint Healthcare Services: Criteria and Fees:

Adult and Family Nurse Practitioners evaluate and treat acute and chronic diseases via diagnosis, disease management, health promotion, and health maintenance protocols and practices. Physical Therapy services promote the mobility and functionality of patients with impairments and disabilities. These remediation techniques are provided by licensed Physical Therapists for a variety of acute and chronic pain conditions and for pre and post-op surgical needs.

        While visiting our practice, patients may encounter advanced standing students who are currently enrolled and earning their Doctorate of Nursing Practice and Doctorate of Physical Therapy degrees. The UHWC provides these upper level students the opportunity to apply their clinical skills in an patient care environment that supports clinical and academic excellence. The Urban Health and Wellness Center provides outpatient healthcare services to UM-Flint students, faculty, our community, and members of the Genesee Health Plan. Services Fees Hours of Operation

 

Visitors Enrolled with GHP Fee:

$0.00 (co-pay) with GHP Plan A for Primary Care or Physical Therapy
$3.00 (co-pay) with GHP Plan B for Primary Care or Physical Therapy

For currently enrolled GHP community members, healthcare coverage is provided for adults aged 19-64 with limited income and no other insurance. Please call our office with further questions about enrolling. *Additional fees pertain to certain additional services.

Student Fee:

$25.00 per visit Primary Care
$25.00 per visit Physical Therapy

For currently enrolled UM-Flint students, there is a fee of $25.00 per primary care or physical therapy visit, as well as GHP Enrollment if not insured. *Additional fees pertain to certain additional services.

Faculty & Staff Fee:

$40 per visit Primary Care
$40 per visit Physical Therapy

For Faculty and Staff at UM-Flint, there is a fee of $40.00 per primary care or physical therapy visit. *Additional fees pertain to certain additional services.

General Visitor Fee (All Other Visitors):

$40 per visit Primary Care
$40 per visit Physical Therapy

For the Greater Flint and other Community Members, there is a fee of $40.00 per primary care or physical therapy visit: (The Genesee Health Plan-GHP-may provide basic healthcare coverage for adults aged 19 to 64 with limited income and/or no other health insurance options-feel free to call the number below for information on how to apply, or visit the GHP website). *Additional fees pertain to certain additional services.
