You can make notes on individual responses using the "Show Notes" link on each detailed response view.  See "Working with Responses" for details.

If you experience problems with FormAssembly views in Internet Explorer, especially while using the FormBuilder, then you may want to consider using Mozilla Firefox.  Firefox does not allow the preview for a form to overlap with the editing tabs, and may make it easier for you to navigate FormAssembly.

To make sure you know what information is considered personal/private please refer to the guidelines outlined by UM-Ann Arbor.  These guidelines, and information on how to safely work with personal information, can be found at: http://safecomputing.umich.edu/privacymatters/ .

Before you, or others, can access your completed form you must make sure that you set it to "Use SSL encryption (secure form)".  To set your form to use SSL encryption please follow the directions found in the Display & Processing section on the "Form Publishing Options" page.

FormAssembly is a wonderful solution to creating forms, but it might not be right for everyone.  If your form requires some form of authentication (for example, asking students to log in using their UMID/SIS PIN before filling out the form) or if you need a complicated back end for data management - FormAssembly isn't for you. 

When FormAssembly simply won't do for your form, you may need to consider developing a form individualized with ASP code and direct access to the SQL database.  The following links provide helpful information about developing ASP based forms:

To have a table created in the SQL database for your form please contact Debi Rowden at (810) 762-3123. 

**To access this link you must have a UM-Flint uniqname and LAN password.  Log-in using youruniqname@umflint.edu and your LAN password (the same one you use to check your email).

The FormAssembly system is set up in a way that users cannot hit the back button and then submit the information again without refreshing the page or reloading the form. This is not an error, but a functionality of the system which goes not allow resubmission of the same data. If you still have problems after refreshing or reloading the page, completely close out of your browser and then restart it.

Instead of using the "My Responses" tab, use the "My Forms" tab. To do this, find the form you need to report on and click the number under the "Responses" columb to view the responses for that form.
